RFU Referee Society Injury Report Form

This RFU form is for use by Society referees to report any serious injury sustained by a player during a match at which the referee is officiating.

The Society's guidance on dealing with serious injuries during a match and subsequent reporting is published in the Handbook (section 5.1) and reproduced here on this website.

If in doubt about the seriousness of an injury sustained by a player, the report form should be completed. For advice on completing the form, please contact the Honorary Secretary.
