Match Official Abuse Form Levels 5 and Below

This is the latest version of the RFU Match Official Abuse Form (issued for the 2024/25 season). Please destroy all previous version of the form and use this one.

The Society has produced a Guide for Referees on Match Official Abuse, which describes categories of abuse and appropriate responses including the use of red cards. It complements the guidance on match official abuse provided by the RFU .

Copies of completed forms must be sent by email to the:

  1. Referee Society Disciplinary Officer at redcardandabuse.madref@gmail.com
  2. Disciplinary Secretary of the Constituent Body of the club concerned.
