Red Card Form for Adult Levels 5 and Below

This is the latest version of the Red Card Form for men's matches level 5 and below and women's matches level 2 and below (2025/26 season). Please destroy all previous versions and use this one.

There is a separate form for red cards given in Age Grade matches.

The Society's guidance on sending off players and writing red card reports is published in the Handbook (section 5.1) and reproduced here on this website. The RFU have also provided a one-page Discipline Report Writing Guidance document.

Copies of completed forms must be sent by email to the:

  1. Referee Society Disciplinary Officer at redcardandabuse.madref@gmail.com
  2. Disciplinary Secretary of the Constituent Body of the club to which the player given the red card belongs.
