Red Card Form for Age Grade Matches
This is the latest version of the Red Card Form for Age Grade matches (2025/26).
Please destroy all previous versions and use this one.
The Society's guidance on sending off players and writing red card reports is published in the Handbook (section 5.1) and reproduced here on this website. The RFU have also provided a one-page Discipline Report Writing Guidance document.
Copies of completed forms must be sent by email to the:
- Referee Society Disciplinary Officer at redcardandabuse.madref@gmail.com
- Disciplinary Secretary of the Constituent Body of the club to which the player given the red card belongs.
