Match Officials Expenses Invoice (Claim Form)

This form is for use by match officials to claim travel expenses from clubs that are "cashless". Currently this is the case mostly for some schools, colleges and universities.

Match officials should give the completed and signed form to the appropriate club representative so that an online payment can be made direct to the match official's bank account.

Please refer any problems with receiving expenses from clubs to Allan Townsend.
