This form is to be used to apply for a Disclosure Barring Service (DBS) check. It also specifies and records the identity documentation that is required.
The Society's Safeguarding and Child Protection Policy requires that from the start of the 2020/21 season all match officials who officiate at U18's matches (i.e. those for players under the under of 18) must be DBS checked.
