Match Abandonment Form

This is the latest version of the RFU Match Abandonment Form (season 2024/25).

Please destroy all previous versions of this form and use this one.

The Abandonment Form is to be filled in by the match official when he/she abandons a match for disciplinary reasons.

Copies of the completed form must be sent to the Society Hon. Secretary and the CB Disciplinary Secretary (see page 14 of the 2022-23 handbook for contact details).
